As a professional educator, it is your responsibility to ensure that all IEP template forms are individualized and reflect the accurate representation of your respective SELPA/District paper-based forms. Working With Templates
With six years of experience in using form recognition software, your paper-based IEP forms are professionally scanned and edited into electronic templates that can be downloaded from this web site. The templates are available in two formats (PC and Mac). What makes this web site unique is how quickly we can respond to our teacher's needs when it comes to refining/updating IEP forms and making them available for download on this web site. Most refinements and updates can be done within 24 hours.
Choosing Template Format:
If you are using a PC we recommend using the PC format. This format is windows-based and can be downloaded without the need of any special software. You can also use 5.0 Filler if you have the appropriate 5.0 Filler software on your computer. Learn more about 5.0 Filler by clicking here. Portable Document Format (PDF) is also available and is recommended for Mac users. However, you will need to have Adobe 5.0 on your computer to open up and use the template. For more information on Adobe, read the information below under Mac users.

Upgrade your templates with Filler 5.0 for PC Users
For PC Users:
| You can now do your IEP documentation on the computer by clicking on each form and selecting "save this program to disk". I recommend saving to your desktop folder(s). Then you can open it up, type your form and "save as" for each one of your student's IEP forms. Click on the images to the right to learn more about creating a folder on your computer desk top and organizing your IEP Templates. Print out the "Help-Aides" if you need to use them. For Macintosh Users:
You will need to download the forms in a Portable Document Form (PDF) format. If you desire to just read the form, type it online, and just print the form, down load Adobe Acrobat Reader 5.0 free of charge. Unfortunately, Adobe Reader is a "read only" software which means you will not be able to save and edit your forms, which is the real advantage of using IEP templates. To save, edit and print your IEP in a PDF format, you will need Adobe Acrobat 5.0 Approval which will enable you to save, edit and print your IEP forms in a PDF format.
Click on the below table to locate your IEP template and then you can download it to your computer. You can also scroll down past the yellow table to find the template with the full title. When you scroll down past the yellow table, remember, PC users click along left column and MAC users click along right column.
